Per estalviar el dolor a la seva dona, la Katherine, accepta adoptar un altre nen nascut a la mateixa clínica, la mare del qual ha mort durant el part. La Katherine, que ignora el canvi, es lliura amb tendresa a la cura del seu suposat fill Damien. Però, mentre el nen creix, també ho fa el terror que l’envolta.
